The following forms are processed via an online workflow. You may check the status of these submitted forms on your dashboard here:

• Change of Academic Program (adding/dropping major, minor or concentration). Graduate students should contact their program director.

• Exception to University Policy(supporting documentation must be provided)

  • Enrollment: Currently Enrolled Students (PROWL > Student Records > Enrollment Verification)
  • Enrollment: Students Enrolled before Fall 1988 (National Student Clearinghouse)
  • Enrollment: Third Party Requests (National Student Clearinghouse)
  • Enrollment: Pre-Term Enrollment
  • Pending Degree Date (students who have applied for degree in PROWL)
  • Student-Expected Degree Date (student self-reported future degree date)
  • International Students/International Purposes
  • Internship Credit

  * Students who wish to request a degree audit adjustment must see their advisors. Advisors or GR program directors initiate the process for a degree audit adjustment.

  ** Only faculty may initiate Grade Revision requests. Please consult with your faculty member regarding grade concerns.
